2. Applicants do not need to guarantee the legality, validity, and authenticity of the materials they submit.
3. Applicants must fill in the form neatly in Chinese with a pen, brush or signature pen.
4. When applying to change the "Advertising Business License", in addition to submitting this form, the applicant must also submit the following application materials: (1) The original and copy of the original "Advertising Business License"; (2) Certification documents related to the change of advertising business scope, unit name, legal representative (person in charge), and business location. The above application materials should be on A4 paper.
