Resident health record management system

"Resident Health Record Management System" is a set of comprehensive management software developed in full compliance with the "National Basic Public Health Service Standards (2009 Edition)" of the Ministry of Health and is fully in line with the current reality in China for township health centers, community health service centers, village clinics and community health service stations. This system is seamlessly integrated with the hospital management system (charge management [outpatient charges, inpatient charges], drug management [drug library management, pharmacy management], doctor workstation, nurse workstation, medical technology management, medical card management, etc.) to share data and support the management of resident health information cards. Medical units can fully realize all functions of file management and diagnosis and treatment services. This software is currently the software that truly realizes the six-in-one of township health centers (communities), real network management, and network query.
​​​​The resident health record management system is centered on the individual, with the family as the unit, and the community as the scope. This system aims to centrally store patient medical data by establishing a community health service computer network, and implement township health centers, community health service centers and community health service stations, or Stations transmit and exchange information with each other, fully realize the sharing of data resources through disease monitoring, statistics and analysis of causes of death, realize the informatization of health service medical management, and build an end-user access tool that can browse, report and come with a decision support system, thereby improving the level of health service management.
The contents of this system include: health record management of urban and rural residents, health education, health management of children aged 0 to 36 months, maternal health management, health management of the elderly, vaccination, health management of patients with hypertension, health management of patients with type 2 diabetes, and management of patients with severe mental illness. The service objects, content, processes, requirements, assessment indicators and service record forms of various public health service projects stipulated in the specifications are all implemented in the software one by one.
(1) Contents of resident health records
Resident health records include basic personal information, health examinations, key population health management records and other medical and health service records.
1. Basic personal information includes basic information such as name, gender, and basic health information such as past history and family history.
2. Health examination includes general health examination, lifestyle, health status and medication for diseases, health evaluation, etc.
3. Health management records of key populations include health management records of various key populations such as children aged 0 to 36 months, pregnant women, the elderly, patients with chronic diseases and severe mental illnesses required by the national basic public health service project.
4. Other medical and health service records include other admission records, consultation records, etc. besides the above records.
5. In rural areas, on the basis of residents' personal health files, basic information and changes of family members can be added, as well as information on family members' main health problems, socioeconomic status, rural household kitchen and toilet usage, poultry stall settings and other information.
6. The system is based on the country's unified administrative division coding, with towns (streets) as the scope and villages (neighborhood) committees as units. It adopts a 16-digit coding system or a 17-digit coding system according to the actual conditions of different regions. The software automatically compiles unique codes for residents' health files.
7. The system supports resident health information card management. The resident health information card management is used to facilitate the annual physical examination, personal file inquiry, and health examination results; if the company's hospital management system is upgraded, the resident health information card can be used by residents to register, pay, seek medical treatment, check up, collect medicines, check out, inquire, etc. in the hospital.
8. The system supports data reporting and reception. Resident health records are reported by village health centers (community health service stations) to township health centers; health centers (community health service centers) report them to the health bureau.
