​​ 2. Purpose of request. On the basis of describing the facts and demonstrating the reasons, put forward a specific and clear purpose of the request. It is best to submit specific requests for consideration by the people's court in accordance with several enforcement measures stipulated in the law.

​​ 3. In the party column, indicate the natural situation. Natural persons must list their name, age, ethnicity, work unit, and address. The legal person or other organization must list its name and domicile, as well as the name and position of its legal representative or person in charge.
